Eligibility

To be eligible for coverage as a Policyholder, you must:

AGE REQUIREMENT

Be between 17 and 45 years old at application.

RESIDENCE RULE

Reside outside your Home Country.

HEALTH REQUIREMENT

Be in good health on the Policy Effective Date—no ongoing treatment, hospitalization, or confinement.

DEPENDENT COVERAGE

If also eligible as a dependent under another policy, you may only be covered once.

Student & OPT Eligibility

Be a non-US citizen; Be a student at a US educational institution; Be enrolled in school for at least one (1) academic year; Agree to work part-time if you want pre-completion Optional Practical Training (OPT); (v) Agree to work in a job related to your field of study; Have no criminal record during your time in the United States; Be eligible for an Employment Authorization Document (EAD); (viii) Not have completed a twelve (12) month, full-time Curricular Practical Training (CPT); Not be enrolled in an English as a Second Language (ESL) course.

No U.S. Residency

Have not obtained residency status in the United States.

PRIOR COVERAGE RULE

Previous WellAway coverage only allowed if new policy has same or less coverage.

VALID VISA

At all times during the Policy Period, comply with the specific requirements and regulations of your program for which you have obtained your visa.

VALID DOCUMENTS

Hold a valid passport, a valid F-1 visa, and a valid Form I-20.

PARENTAL CONSENT

If under 18, a parent or guardian must complete necessary forms and sign the application.

MARRIAGE CLAUSE

If marriage to a U.S. citizen and eligibility for status adjustment ends plan eligibility.
